Digitized Care Coordination Workflows at Horizon Corporations

The Horizon Corporations, which includes Horizon Health Services, Horizon Village, and Horizon Health Alliance, is one of WNY’s largest not-for-profits. With over 875 employees, Horizon is a fully comprehensive behavioral health treatment provider operating 199 inpatient beds for addiction treatment and outpatient counseling and psychiatry practices throughout Erie, Niagara, and Genesee Counties. Horizon provides treatment, recovery and support services for children, adolescents, adults and families affected by mental health and substance use disorders.

Celebrating their 50th year serving the community, Horizon was founded in 1975 and has been voted one of the “Best Companies to Work for in New York” for 17 consecutive years, awarded one of WNY’s Healthiest Employers for the last 4 years and in 2025 recognized as a Top Workplace in America.

At Horizon, the vision to be best-in-class to work and receive care is more than a strategic goal, it is a lived commitment that shapes every facet of the organization.

Horizon’s patient centric approach ensures that care is not only clinically excellent, but person centered.

A steadfast commitment to bold and imaginative strategies has positioned Horizon as a forward-thinking leader in behavioral health. By continuously evolving to meet the progressive needs of both their workforce and the communities they serve, Horizon exemplifies what it means to be a modern healthcare provider rooted in compassion, excellence, and innovation.

Horizon’s embrace of digital solutions reflects a broader organizational focus and that is to lead with vision, adapt with purpose, and serve with heart. Their investment in technology is more than a workflow upgrade but a strategic move toward a more connected, responsive, and resilient healthcare system.

3 Key Success Pillars

5-10 Minutes Saved on Form Completion Process, Per-Patient.

The Horizon team cares for tens of thousands of patients every year. Before Fluix, each patient would need to attend in-person to complete and sign paper forms, which were printed, completed, prepared for signature, and then manually filed by Horizon’s administrative team. Now, documents can be securely emailed to patients in any location, signed by them, and sent back to Horizon within minutes – saving both time and costs previously associated with printing and completing paper versions.

Remote Digital Document E-Signature

Horizon requires specific documents to be signed by their patients before they can start to coordinate care with other providers or insurances. For some patients, needing to physically sign those documents at one of Horizon’s locations wasn’t always immediately possible, and potentially delayed them receiving care. With Fluix, patients are sent all of the necessary documentation in secure digital format, which they can sign from their phones or other devices at home and immediately return to Horizon so that their treatment can begin.

Compliance and Patient Confidentiality

Horizon employs 875+ FTEs across 22 outpatient locations, including a team of approximately 50 admin assistants. Fluix helps the company to ensure that all regulations are up-to-date, and all paperwork is signed and in patients’ charts. Further, the company now has a secure digital document repository that can be instantly accessed for reference or audit – eliminating their reliance on physical paper copies of documents containing sensitive patient data/information, and the risk of loss or damaged paperwork.

Milestone Moment

“Fluix is the first system we’ve had for electronic signature and sending forms, and it helped us to keep our services going without interruption during the COVID-19 pandemic.”

Sarah Just, Corporate Director of Administrative Operations at Horizon Health Services
